I'm pretty sure the answer is going to be "no", but I'll try anyway...
Is there a way to associate a keyboard shortcut with a plug-in so you don't have to go through multiple levels of cascading menus to invoke it?
Alternatively, is there a way to create a toolbar button for it? Or a custom menu item?

There is no way to assign a keyboard shortcut, but you can use standard menu keys, like Alt C N, to quickly navigate the menus.
You can use the Options | Tool Bar command to add a button for a plug-in to the effect tool bar. In GoldWave v5.12 you can right-click on a an effect tool bar button to select and use a preset it directly without displaying nested menus or the effect window.
